Data subjects seeking to exercise their right to review, correct, alter, or delete their personal information or preferences pursuant to the Shred Vault Privacy Policy, should complete and submit this form.

Shred Vault will respond and/or comply with legitimate, reasonable data subject requests within 30 days from the submission date.

I understand that I may be prosecuted for submission of false information or for repeated submissions of irrelevant and/or nuisance requests in an attempt to place an undue or unreasonable burden upon Shred Vault.

I understand and agree that repeated, fraudulent, or unreasonable requests, including requests that do not facilitate reasonable verification of the submitter’s identity are grounds for disregarding or denying this request.

I understand and agree that identity verification measures for purposes establishing the legitimacy of this request may require Shred Vault to contact me and/or for me to provide more information.

I understand and agree that my failure to cooperate with reasonable attempts to verify the legitimacy of this request are reasonable grounds for disregarding or denying it.

Upon submission, a verification notice will be sent to the email address associated with prior Shred Vault interactions. Failure to verify this request within 72 hours will result in its deletion.
